TO SUBMIT A CLAIM, please call 785-537-1600 and ask for the Claims Department.

Please be prepared to provide the following information:

• Insured's Name
• Contact Phone Number
• Date of Occurance
• Time of Occurance
• Police Report Number (if applicable)
• Other Party's Information
